Medical History Sheet, from 3 January 1916 examination, notes: i) height 5’4”, weight 160 lbs, physical development - good; ii) 1 vaccination mark on left arm from 15 years ago; iii) scar on right side of forehead, the result of an explosion; iv) slight defect but not sufficient to cause rejection - slight varicose; v) vaccinated 4 May 1916; vi) inoculations given 14 May 1916, 8 June 1916, and 20 July 1916

c. No Memorial Cross issued. Plaque (serial number 752039) and scroll not issued - records indicate “unable to determine next of kin”

d. Pay records indicate surname was initially recorded as “Hagar”; corrected to “Hager” on the May 1916 paylist.
