Born 2 March 1895 in Harwood, Ontario. Son of George Cook, of 128, Melbourne St., Lindsay, Ontario. He enlisted on 22 Sepember 1914. He listed himself as farmer and stated he was single.
Taken prisoner by the Germans in July of 1915. Repatriated February 24, 1918 and immediately hospitalized. Sent back to Canada for further treatment but died of tuberculosis in hospital at Kingston.
