Circumstance of Death report gave the following:
"This soldier was one of the gunners riding inside of a tank to the final objective, during an attack. The tank was put out of action by an enemy shell and set on fire. During the evacuation of the tank, the crew came under heavy machine gun fire and Private Bernard was instantly killed".
