Son of John Royal Lewis and Sarah E. Lewis, of 337, 13th Avenue East, Calgary, Alberta.
He originally enlisted with the 4th Overseas Universities Company and joined The Royal Canadian Regiment in the field on August 5, 1916 where he served for one month until he was killed in action.

Dark complexion, brown eyes, dark brown hair. Scar from appendectomy on September 1914. Had his medical at Sarcee Camp. Had previous experience with the 103 Regiment Calgary for six months.
On February 15, 1916: he had influenza. He was hospitalized until March 11, 1916 at Shorncliffe -- Moore Barracks Hospital. He also had typhus on March 2, 1916. He had surgery for a hernia he developed...May 13, 1916. (See images for more details.) He had a slight varicocele (left side of scrotum).
