There are two attestation forms for H. B. Cooke. The earliest one was for May 12, 1915, and was signed by ? Macdonald, Matron-in-Chief for D.M.S. Canadian Contingents. The data from this attestation was used to update this record.

The second attestation dates to November 11th, 1915. It was signed at "Mudros West", which probably refers to Moudros on the island of Lemnos, Greece. This attestation was signed by an officer of No. 1 Canadian Stationary Hospital.
