This rank is given in the LG citation of the award of the Royal Red Cross in June 1918.
The citation specifically says "(Lt. C.A.M.C)" and states currently "Can. Nursing Service, Can. Spec. Hospl., Lenham, Kent". I can't find any other mention of a Canadian Nursing Service as a force though.
